Referral / Intake Coordinator

We are seeking a highly organized and relationship-focused Referral / Intake Coordinator to manage the full referral and intake process for residential programs. This role serves as the primary point of contact for referral sources, families, care managers, and internal teams regarding residential vacancies and admissions.


This position requires experience in AFH, CBRF, long-term care, behavioral health, or related settings, as well as a strong understanding of DHS 83 and DHS 88 regulations related to admissions and discharges.


Key Responsibilities


Referral Management

  • Receive, track, and respond to all residential referrals

  • Communicate promptly with referral sources, families, and guardians

  • Gather and review required documentation (assessments, service plans, medical records, behavioral history)

  • Complete initial screenings to determine eligibility and appropriate placement


Intake Coordination

  • Schedule and coordinate tours, assessments, and pre-admission interviews

  • Communicate admission details with internal teams including nursing, billing, and residential staff


Documentation & Compliance

  • Maintain accurate and confidential records in compliance with DHS 83 and DHS 88

  • Ensure compliance with all licensing, regulatory, and organizational standards

  • Maintain and update tracking systems related to referrals, openings, admissions, and occupancy

  • Prepare reports on referral trends, occupancy, and program capacity as requested


Communication & Collaboration

  • Partner with residential management to determine availability, staffing considerations, and placement fit

  • Build and maintain strong relationships with care managers, MCOs, hospitals, and community partners

  • Participate in team meetings, staffing discussions, and continuous improvement efforts


This position will be in-person at the CCLS Watertown office 3 days a week.
