Creative Community Living Services is a leading agency providing in-home support for adults with disabilities. We are looking for a nurse to provide administrative as well as hands-on care for our clients in Stevens Point, Wisconsin Rapids, Neillsville, and Marshfield, WI.

What You Will Be Doing, besides working with great people:

• Ensures consistent quality client care

• Provides training and education to staff on an ongoing basis.

• Assists in the maintenance of residents’ charts and review of documentation performed by caregiving staff.

• Provides medically orientated task training, delegation and oversight to staff on an as needed basis.

• Ensures Care Plans are up to date and reflective of a change in condition for each resident.

• Oversees and manages the continuity and consistency of medication including daily/weekly audits including psychotropic reviews.
